The first installment of ONE QUARTER (3 months), plus an initial REGISTRATION FEE of $50 per new patient, is due upon execution of contract.
Patients may pay on quarterly or monthly schedules. Monthly payments must be made via EFT (Electronic Funds Transfer) with your Debit/Credit (VISA, MasterCard or Discover) cards on an automatic scheduled monthly basis with an authorization form completed and on file at our office
